29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch: On or about October 31, 2014, and at times prior thereto, work rests were not adjusted with a maximum opening of one eighth inch, exposing employees to struck-by hazards: a) Between rows-E and F, the DEWALT Model DW 756, 6-inch Bench Grinder, Serial No. 2009 51-YL37941, opening was approximately seven-eighths of an inch. b) Inside the Maintenance Tool Room, the 1/2 HP Baldor Bench Grinder Cat No. 712/Serial No. W584, opening from the wheel was approximately five-eighths of an inch on both sides.
