29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about October 8, 2014, and at times prior thereto, the employer did not ensure that employees were provided with fall protection equipment while performing work on top of elevated work locations. a. In the Shot Blast Department, employees were required to access the top of the shot blast booth to perform work, exposing employees to a fall hazard of approximately 10 feet. b. In the Heat Treat Department, employees were required to access the top of 5 ovens to perform work, exposing employees to a fall hazard of up to approximately 11 feet.
