29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Work Site: 434 Rockaway Valley Road, Boonton, NJ. A daily inspection of the trench was not made by a competent person. Trench had visible deficiencies, such as but not limited to, no protective cave-in system with maximum depths of 9 to 13 feet, a lack of safe egress with the exit being a 41 foot travel distance and no utility mark outs. Violation observed on or about 10/01/2014.
