29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: On or about October 01, 2014, and at times prior thereto, the employer allowed employees to use makeshift fasteners,. The employer did not ensure the load was secure where makeshift fasteners were used, such as but not limited to: a) A shop made lifting eyebolt, b) A shop made lifting lug. Employees were exposed to the hazard of being struck by/crushed by a falling load.
