29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling: a) 15 Thatcher Road, Dayton, NJ 08810. The employer allowed his employee to work in close proximity to energized equipment when this equipment was not installed in accordance with any instructions included in the listing and or labeling. The equipment is rated at 208/230 volts and was operating off a 110 volt system exposing the employees to shock and or electrical burn hazards. Incident occurred on or about 09/11/14.
