29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: The employer is failing to protect employees from struck by hazards associated with operating forklifts without having been certified at least once every three years as outlined in this subpart. This was most recently evident during the inspection of the facility located at 2500 Old Hadar Rd. Norfolk, NE 68701; where the employer has employees operating forklifts throughout the facility without having recertified their forklift operations within the past 3 years.
