29 CFR 1910.119(g)(2): Refresher training was not provided at least every three years, and more often if necessary, to each employee involved in operating a process to assure that the employee understood and adhered to the current operating procedures of the process. The employer, in consultation with the employees involved in operating the process, shall determine the appropriate frequency of refresher training: The employer did not consult with employees concerning the frequency of their need for refresher training. Employees were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
