Text For Citation: 01 Item/Group: 006 Hazard:
29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. These safe work practices shall apply to employees and contractor employees: a) Old Acid Room Area - Employees are exposed to a potentially catastrophic release of Formaldehyde from the employer not developing safe work practices for activities that required opening process equipment or piping or activities that required the isolation of hazardous energies (lockout/tagout) associated with the Formaldehyde Loading System, on or about September 26, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
