29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: The employer does not ensure that ladders are not used on scaffolds to increase the working level height of employees. This violation occurred on or about April 28, 2012, on the interior west wall of a residential home where employees were exposed to a fall hazard of approximately 20 feet when spraying foam insulation in the living room from a 12 foot extension ladder that was set up and used on the second tier of a mobile scaffold to gain additional height.
