The employer's machine-specific energy control procedures do not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: 29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section: A recent example of this occurred on 10/25/11 and prior, when: a) In the establishment, Cream Tank #3 Lockout/Tagout(LOTO) procedure did not clearly describe all machine energy sources and the techniques to isolate them in order to protect employees performing maintenance/servicing activities inside this tank from hazards such as, but not limited to: struck by a moving agitator inside this tank, contact with chemicals and engulfment by a liquid entering this tank. To abate this violation, the employer shall develop and implement a machine-specific Lockout/Tagout(LOTO) procedure to properly isolate Cream Tank # 3. For example: Develop steps to Lockout agitator's electrical source, bleed stored liquid inside tank and remove/blank/cap piping sections to prevent contact with chemicals and engulfment. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, within 30 days of the date it receives this citation.
