29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address the engineering and administrative controls applicable to the hazards and their inte rrelationship, such as, appropriate application of detection methodologies to pr ovide early warning of releases. The employer does not ensure that the process h azard analysis addresses the engineering and administrative controls applicable to the hazard. a) This violation occurred at the Lion Copolymer, LLC facility lo cated in Baton Rouge, LA on or about October 13, 2010 in the Utility Pump House Area where the employer did not address the consequences of having enclosures on valves, to prevent leakage, in the ammonia refrigeration piping system when Lio n Copolymer conducted the 2009 Process Hazard Analysis (PHA) Revalidation, expos ing employees to potential loss of containment of anhydrous ammonia. This expose d employees to fire and explosion hazards. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure PHAs address th e engineering and administrative controls applicable to hazards and their interr elationships.
