29 CFR 1910.119(e)(6): The employer did not ensure after the initial process hazard analysis that the process hazard analysis was updated and revalidated at least every five (5) years by a team meeting the requirements of 29 CFR 1910.119(e)(4): a) On or about February 24, 2010 - the initial Process Hazard Analysis (PHA) available in the plant was not updated and revalidated every five years (2002 and 2007) after the completion of the initial PHA (1992). The employer did not ensure that the PHAs performed in 1992 were revalidated by a team meeting the qualifications in 1910.119(e)(4).
