29 CFR 1910.119(e)(5): Equalizer Midwest, Inc. - Hastings, NE - The employer did not establish a system to promptly address the process hazard analysis team's findings and recommendations and assure that the recommendations are resolved in a timely manner. Multiple action items from the 2005 PHA were not resolved and documented in a timely manner including, but not limited to: a)Devise and implement valve cycling program. b)Determine secondary containment requirements for ammonia storage at the time of construction. c)Ensure that all lines have thermal expansion protection. d)Investigate the need for safeguards at main road and road south of tanks to protect cold service line. e)Upgrade mechanical integrity program to include labeling and indentifying of valves and piping. f)Investigate installation of automated discharge into 1st stage to provide overpressure protection to Flash Drum. g)Install solenoid valves on 1" and 2" lines to Flash Drum or establish SOP for operation. h)Investigate safe locations for PSV outlet piping (D-2b Flash Drum, Receiver and liquid line exit FD). i)Consider revising outlet piping of PSVs to be protective of personnel. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
