Text For Citation: 01 Item/Group: 006 Hazard: REFINERY
29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in t he process complied with recognized and generally accepted good engineering prac tices (RAGAGEP): This violation was observed on or about 1/19/2010, where the em ployer did not comply with RAGAGEP when it failed to protect employees inside an inadequately protected structure that was exposed to explosion and fire as a re sult of an Highly Hazardous Chemical release from process equipment. The primary office/crew quarters structure, loca ted on the North side of the cellar deck of the EC2 platform that was occupied b y three employees was totally destroyed when an explosion and resulting fire occ urred on the EC2 platform. The structure was of fiberglass and wood construction . Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the cita tion, the employer must submit documentation showing that it is in compliance wi th the standard, including describing the steps it is taking to ensure that prop erly constructed office/crew quarters are provided to platform personnel as desc ribed in API RP14J.
