29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not effectively address human factors: a) Sunoco, Inc. Marcus Hook Refinery, 10-4 FCCU - The employer did not ensure that the 1996 PHA effectively addressed human factors, as evident by the fact that the PHA did not address the inaccessible location, lack of position indicator, and inability of the operators to visually or tactually verify the position of the 10-4 Wet Gas Compressor By-Pass valve. Condition noted on or about March 10, 2009. b) Sunoco, Inc.
