29 CFR 1910.119(d)(3)(i)(B): Process safety information pertaining to the equipment in the process did not include the piping and instrument diagrams (P&IDs): a- Hess Port Reading Refinery, FCCU - RV set pressures for D-507 Main Column Overhead Receiver were marked incorrectly on P&ID 62-D13. RV-D-507-1, RV-D- 507-3 and RV-D-507-4 were recorded on the P&ID at 23 psi, 23.6 psi and 22.5 psi when the correct settings should have been 23.6 psi, 22.5 psi and 23 psi, respectively. Condition noted on or about 03/13/09. b- Hess Port Reading Refinery, FCCU - RV set pressures for T-501 Main Column were marked incorrectly on P&ID 62-D12. RV-T-501R-1, RV-T-501R-10, RV-T- 501R-2, RV-T-501R-3, RV-T-501R-5, RV-T-501R-6, RV-T-501R-7 and RV-T-501R- 8 were recorded at 27 psi, 28.3 psi, 28.35 psi, 28.35 psi, 27.6 psi, 27.5 psi, 28.3 psi and 28.3 psi when the correct settings should have been 28.3 psi, 28.35 psi, 28.3 psi, 28.3 psi, 27 psi, 27.6 psi, 27.6 psi and 27.6 psi. Condition noted on or about 03/13/09. d- Hess Port Reading Refinery, Dimersol - Set pressures for rupture disks (RD) associated with equipment RVs to include S-2204A Stabilizer Feed Filter, S-2204B Caustic Rag Layer Filter, R-2201 1st Reactor, D-2202 Water Wash, D-2201 Caustic Wash, R-2202 2nd Reactor, D-2213 Ammonia Mix Drum and R-2203 Finishing Reactor were not indicated on P&IDs. Condition noted on or about 03/05/09.
