29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment to maintain its mechanical integrity, did not follow recognized and generally accepted good engineering practices: (a)In Plant 81 of the facility: The employer did not resolve anomalous piping inspection data by ensuring that piping wall thickness data discrepancies were evaluated. During nondestructive evaluations using ultrasonic testing methods showed that piping section 8102-003-05 TML#3 showed an increase in piping wall thickness from .610 to .700 (90 Mils) from 01/05/00 to 07/22/04. (b)In Plant 16 of the facility: The employer did not resolve anomalous piping inspection data by ensuring that piping wall thickness data discrepancies were evaluated. During nondestructive evaluations using ultrasonic testing methods showed that piping section 0016-034-12 TML#9 showed an increase in piping wall thickness from .345 to .385 (40 Mils) from 12/04/00 to 12/22/003. (c)In Plant 81 of the facility: The employer did not resolve anomalous piping inspection data by ensuring that piping wall thickness data discrepancies were evaluated. During nondestructive evaluations using ultrasonic testing methods showed that piping section 8102-004-01 TML #6 showed an increase in piping wall thickness that was not resolved.
