Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the possibility of the ten inch grinding wheel exploding as a result of side grinding: (a) Facility, Arch Street, Carnegie, PA: On or about March 6, 2008, the Worcester 10" Drill Grinder was being used with a flat grinding wheel instead of the cup grinding wheel that was required. As a result employees were side grinding on the 10" abrasive wheel on the drill grinder. Among other feasible methods and acceptable means of abatement to correct the hazard is to use the correct cup type grinding wheel.ng
