Text For Citation: 01 Item/Group: 006 Hazard:
29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) In the 13, 14 Battery Transfer Area Pad Valve Pit, on or about August 11, 2025 - The employer did not ensure energy isolating devices were operated in a manner to isolate 13 Pad Valve from cryogenically-processed coke over gas while employees cleaned the seat of 13 Pad Valve with water when cryogenically-processed coke oven gas was present in the valve and Underfire lines. As a result, the remaining applicable energy control elements involving LOTO device application (d)(4), stored energy (d)(5), and verification of isolation (d)(6), were not implemented to safeguard employees from the 13 Pad Valve washing hazards. This exposed employees washing the valve, as well as other employees working in or adjacent to the 13, 14 Battery Transfer Area to an explosion when 13 Pad Valve ruptured during washing and released cryogenically-processed coke oven gas from the Underfire lines.
