29 CFR 1910.252(a)(2)(iii)(A): The employer did not provide fire watchers whenever welding or cutting was performed in locations where other than a minor fire might develop: On or about October 22, 2024, a fire watcher(s) was not present for torching work that occurred at the dye machine's outfeed conveyor, where combustible mulch was present on and around the conveyor. Employees were exposed to fire hazards when hot slag ignited nearby mulch.
