29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: On or about October 15, 2024, it was documented that employees engaged in material handling in the yard of Rail Modal Group located at 549 East County Road T, Freemont, Nebraska were exposed to struck-by hazards in that employees operated the Terex Yard Master 05, model HR45-25, serial number TFC45170904 in which the steering wheel was duct taped to replace a mounting pin.
