29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion. (a) On or about October 10, 2024, and at times prior, at 901 North 9th Street, Milwaukee, WI 53203, the employer had not secured the vertical lifelines to a fixed safe point in that the lifelines were secured to a single steel cable anchor point that was wrapped around the parapet wall on the north side of the building. Additionally, the suspension scaffold was also tied back to the same steel cable the vertical life lines were connected to. (b) On or about October 10, 2024, and at times prior, at 901 North 9th Street, Milwaukee, WI 53203, the employer had not verified the vertical lifelines were independent of the scaffold system in that the steel cable that held the lifelines and the steel cable that held the tiebacks were connected by a Crosby 3/4" D-ring shackle.
