29 CFR 1926.1211(b)(2): An employer whose employees have been designated to provide permit space rescue and/or emergency services did not train each affected employee to perform assigned rescue duties. The employer did not ensure that such employees successfully completed the training required and established proficiency as authorized entrants, as provided by �� 1926.1207 and 1926.1208; On or about September 12, 2024, and at times prior thereto, the employer did not train rescue personnel for extraction in a lift station up to 42 feet below grade level. As a result, employees worked in engulfment hazards.
