Text For Citation: 01 Item/Group: 006 Hazard:
29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Molding station #6 - On or about February 17, 2024, the employer did not require utilization of an energy isolating device for maintenance of a cord-and-plug MAC Automation Concepts, Inc. Small Frame Aluminum Belt Conveyor thereby exposing employees to caught in hazards. As a result, an employee suffered a fingertip amputation when servicing the energized and running equipment. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
