29 CFR 1915.12(d)(5): The employer did not certify that the training required by paragraphs (d)(1) through (d)(4) of this section had been accomplished. Bldg 31C20T, Temporary Fabric Shelter: On or before March 14, 2024, employee(s) entered confined spaces such as fuel tanks 1-4, oily tanks, and overflow tank to conduct welding operations. The employer did not certify that each employee was trained before entering a confined or enclosed space or other areas on the LCAC that may have dangerous atmospheres. Employee(s) were potentially exposed to toxic atmospheres.
