29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) On or about 11/23/2023 at a roofing project located at 51 St. John's Parkside Buffalo, New York; employees were not trained in proper recognition of fall hazards and use of fall protection such as but not limited to, wearing fall protection harnesses properly around the shoulders and tightly around the body, using the Dee ring as the connection point for the lanyard, and having proper anchor points on the roof. An employee, wearing a harness but not tied off, was on the roof holding a 4'x 8' sheet of plywood when a gust of wind caught the board, the employee lost his balance and suffered a fatal fall of approximately 50 feet.
