29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Employees engaged in roofing operations located at 842 South Kansas Ave, Hastings, Nebraska 68901, were exposed to fall hazards in that the employer failed to provide training on the recognition of fall hazards related to the use of portable ladders in residential construction. Instances of fall hazard exposure at this jobsite include but are not limited to: i. Employees not extending the ladder a minimum of 3 feet above when accessing and egressing upper landing surfaces. ii. Employees were not maintaining 3 points of contact in that they were carrying material while climbing up and down the ladder. iii. Employees were leaning the center of their body past the side rails of the ladder.
