29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Instance a) On or about August 9, 2023, the employer did not ensure that the Tishken Model 8-MLW Roll Former (Serial 1571-746) on the RF6 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies were not adequately guarded to prevent employees from accessing the zone of danger. Instance b) On or about July 27, 2023, the employer did not ensure that the Tishken Model 10MW2 10-Stand Roll Former (Serial 1601-346) on the RF15 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies did not have any guarding to prevent employees from accessing the zone of danger. Instance c:) On or about July 21, 2023, the employer did not ensure that the Dahlstrom 11-Stand Roll Former on the RF9 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies did not have any guarding to prevent employees from accessing the zone of danger. Instance d) On or about September 1, 2023, the employer did not ensure that the Dahlstrom 9-Stand Roll Former (Serial 862) on the RF10 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies did not have any guarding to prevent employees from accessing the zone of danger. Instance e) On or about August 21, 2023, the employer did not ensure that the Dahlstrom Model 550-7 8-Stand Roll Former (Serial 841) on the RF42 line was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the roll forming dies were not adequately guarded to prevent employees from accessing the zone of danger. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
