29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 16, 2023, the employer did not ensure that a portable extension cord on the ground in the bag and tag area was inspected for damage. The extension cord was frayed with the inner conductors exposed close to the plug end. This condition exposed employees to electrical shock hazards. (b)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 16, 2023, the employer did not ensure that a portable extension cord plug in cord in the Casino Fabrication/Metal Fabrication area, which was plugged into an outlet on a vertical column was visually inspected. The extension cord was frayed with the inner conductors exposed at the plug end. This condition exposed employees to electrical shock hazards.
