Industrial Park Drive, Milton, PA: The employer did not conduct an adequate assessment of the workplace to determine the hazards that were present that necessitated the use of personal protective equipment (e.g., welding goggles and welding helmets) for employees exposed to eye and face injury while performing plasma cutting operations, on or about March 8, 2023. b) 201 N.
