29 CFR 1910.119(l)(1):The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: (a) Building 1, trains 6/7 (Ethanothebaine crude wet process), during hydrogenation: On or about 12/22/22 until 1/19/23, the employer did not implement management of change (MOC) procedures to manage changes associated with continuing to operate vessel H-1024 with a flange leak occurring at the bottom valve of the vessel, exposing employees to fire, explosion, and toxic hazards during operation.
