29 CFR 1910.146(d)(8): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not designate the persons who were to have active roles in entry operations, identify the duties of each such employee, and did not provide each such employee with the training required: The employer was failing to protect employee from hazards associated with entering a permit required confined space.. This was most recently documented on February 16, 2023 at 417 West Railway, Bruning, Nebraska. Employees were entering boot pits for cleaning purposes without having the required training to be an active member of a permit required confined space.
