29 CFR 1910.146(i)(6): The employer shall ensure that each attendant monitors activities inside and outside the space to determine if it is safe for entrants to remain in the space and orders the authorized entrants to evacuate the permit space immediately. a. On or about November 28, 2022 and times prior, the employer did not ensure that the attendant continuously monitored activities inside the confined space while entrants performed tank cleaning in order to identify unsafe conditions.
