29 CFR 1910.146(d)(10): The employer did not develop and implement a system for the preparation, issuance, use, and cancellation of entry permits as required by this section. (a)On 8/15/2022 and at times earlier, in the Coke Hopper, contractor employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy in the hopper at the point of operation for the vibrator because CORE did not develop and implement a system for the preparation, issuance, use, and cancellation of entry permits as required by this section.
