A) FAZTEC Industries - 200 Bloomfield Ave, Staten Island NY 10314 Damaged cable insulation of conductors (exposed copper) operating at 460VAC were not protected with an insulator equivalent to that of cables original insulator or with an insulating device for that purpose. Condition noted on or about August 27, 2021
