29 CFR 1926.1416(d)(3)(i): The crane was not equipped with a functioning anti-two block device: On or about 20 July 2021, at the Estate Donoe construction site, St. Thomas, VI. Employees engaged on construction operations were exposed to struck-by hazards in the event of a sudden free fall of pre-fabricated concrete walls hoisted by a crane which lacked an anti-two blocking device. NOTE: Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).
