29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Entire Facility: On or about 05/30/2021, and at times prior, the employer exposed employees to thermal, electrical, struck-by and caught-in hazards in that, the employer did not adequately provide and/ or document either authorized or affected employee training before employees performed servicing or maintenance on facility and production equipment including, but not limited to, Tank M2.
