29 CFR 1910.119(l)(1):The employer did not establish written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: On or about May 28, 2021, where sulfur dioxide (liquid) was present in quantities greater than 1000 pounds, the employer had not developed and implemented written procedures to manage changes to process chemicals, technology, equipment, and procedures; and changes to facility that affect a covered process. Lack of management of change procedures could result in equipment failures and loss of containment due to failure to: assess the impact of changes, update operating procedures with any changes, to update changes to process safety information, and to train employees on the proposed changes, exposing employees to hazards associated with sulfur dioxide.
