29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service. Location: 1045 Flatbush Avenue, Brooklyn, NY 11226; Roof On or about 05/17/2021 and thereafter, harnesses were not inspected prior to use. Employees wore harnesses during the demolition of a roof and were exposed to a fall of approximately 60 feet.
