29 CFR 1910.178(l)(6):The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a) Facility -wide: On or about 05/19/2021, and at times prior, the employer exposed employees throughout the facility to struck-by hazards in that uncertified employees were allowed to operate the company's powered industrial trucks including, but not limited to, forty- six (46) sit-down fork trucks, including Toyota Propane Forklift Truck Model #: BFGCU30, S/N: 72642; five (5) electric pallet jacks; and one (1) aerial lift.
