29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolated. On or about May 17, 2021, and times prior thereto, South Side, the employer did not provide training to employee(s) on the function of the energy control program, the knowledge, skills required for safe application, usage, and removal of energy controls for the TruLaser 3030, exposing employees to injuries by unexpected start-up or release of hazardous energy.
