29 CFR 1910.421(g): The breathing gas supply system including reserve breathing gas supplies, masks, helmets, thermal protection, and bell handling mechanism (when appropriate) were not inspected prior to each dive. A) The employer was conducting dive operations in a geoduck tract 5 miles east of Port Angeles, Washington at a depth of 70 feet. They were using an airline system. The employer had no documentation of equipment inspection prior to any of the dives performed in this area.
