29 CFR 1926.451(f)(4):Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. a. 136 Summit Avenue Jersey City, NJ Employees performing masonry work from a job-made elevated work platform constructed over an elevator shaft were exposed to a fall hazard of approximately 97ft when the platform contained weakened components such as but not limited to; support beams with cracks and other visual signs of damage which were not removed from service. On 3/10/2021, the elevated work platform partially collapsed while two employees were working from it. One employee fell approximately 97ft to the bottom of the elevator shaft, sustaining fatal injuries. Violation occurred on or about 03/10/2021
