29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a) mixing area upper catwalk in front of mixer number 3; On or about October 27, 2020: the employer exposed employees to electrical hazards in that employees were allowed to walk and work and access a duplex outlet that did not have a cover. b) mixing area upper catwalk in front of stairway access; On or about October 27, 2020: the employer exposed employees to electrical hazards in that employees were allowed to walk and work and access electrical junction box where 3 conduits intersected that did not have a cover. c) mixing area upper catwalk in front of mixer number 2; On or about October 27, 2020: the employer exposed employees to electrical hazards in that employees were allowed to walk and work and access a single pole receptacle and a duplex ground fault circuit interrupter (GFCI) receptacle that did not have a cover. d) mixing area upper catwalk in front of mixer number 1; On or about October 27, 2020: the employer exposed employees to electrical hazards in that employees were allowed to walk and work and access a single receptacle outlet that did not have a cover. e) mixing area upper below the catwalk area; On or about October 27, 2020: the employer exposed employees to electrical hazards in that employees were allowed to walk and work and access a duplex receptacle outlet that did not have a cover.
