29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Russell T Bundy Associates, Inc. - Mansfield, OH: On or about February 11, 2020, the employer did not ensure that the vertical mixer associated with the Americoat reserve tank at the glaze line was adequately guarded to protect employees from rotating parts. Employees that accessed the top of the tank and/or opened the tank lid, were exposed to caught-in injuries created by the vertical pneumatic mixer.
