29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about 1/13/2020, in the production area at 2555 Moreland Avenue, Atlanta, GA: Employees performing servicing and maintenance activities such as but not limited to removing and replacing the blades on the band saw equipment were exposed to amputation and caught-in hazards, in that each authorized employee did not affix a lock and/or tag on the electrical energy source.
