29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Location: 409 East 110th Street, New York, NY 10029 On or about: December 21, 2019 (a) Authorized employees did not lockout, tagout, nor affix lockout tagout devices to energy isolating devices prior to performing maintenance on parking lifts. Employees were exposed to uncontrolled energy sources while performing maintenance on park lifts.
