29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about December 3, 2019 the employer failed to ensure that effective machine guarding was in place to protect employees from the hazardous area of the Cincinnati 400 ton press brake. The press brake was operated by foot pedal without the use any other type of additional guarding. Additionally, the rear of the press brake was not guarded and able to be accessed by personnel.
