29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s): At 818 Lone Star Drive, O Fallon, Missouri, on 09/24/19, the Heli Forklift, m/n CPY25-TY5, s/n 230251E0507, was operated with the following deficiencies: a. A missing horn mechanism. b. A broken seat belt mechanism. c. A damaged seat. d. A set of non-operational lights. Such deficiencies could result in serious caught-by or struck-by hazards for the operator or other employees working nearby.
