29 CFR 1910.119(d)(3)(i)(D): The employer did not maintain an accurate listing of the location, function, and inspection information for all Relief Valves (RV) within a covered process: On or about July 23, 2019, and times prior thereto, the employer failed to maintain an accurate and updated list of the location, function and inspection information for the relief valves within the covered process. The employer processes over 10,000 lbs of an unscented flammable propane mixture for the production operation. Employee(s) are exposed to the hazards associated with propane and butane such as burns, fire, explosion and asphyxiation.
